              Do you know of any way to fix a 2019 1099 R distribution error. There was a CD Maturity that should have been rolled over into another IRA product. When the bank sent the check it got deposited into the recipients savings account. She is 94, any ideas how to fix this and avoid paying taxes on monies that should still be in and IRA?

    There are specific timeframes that must be followed in order to resolve that issue, in which the funds must be reinvested into another Qualified IRA  account. Of that was not complied with at all then the best bet would be to speak with the bank, explain their error, and see if they will correctly roll it into the CD  it should have been put into in the first place, which most financial institutions do automatically. Suggest trying  to explain possible age and hardship issues caused by bank error . Maybe they will help you.

    You should consider Rev Proc. 2016-17 for self certification of missing the 60 day rollover window, and report the CD proceeds as a rollover on the 2019 tax return.. This means giving the letter described in the procedure along with a check for the full amount to the bank. Not sure if the bank made an error by simply not rolling the CD into another CD within the IRA,. Usually you have to request a payment from an IRA. Was it the RMD?

    I've used this procedure a few times, with no issues.

    Now, in the new Revenue Procedure 2016-47, the IRS has announced a new and greatly expedited process to receive relief and the ability to fix a botched IRA rollover. If the 60-day deadline was missed for any of 11 different reasons specified in the new guidance – from a natural disaster to an illness or death in the family to an error of the financial institution – the individual can now complete a late rollover contribution as soon as practicable, and simply provide the financial institution a "self-certification" that the rollover was permissible based on one of the specified reasons. And the IRS has even provided a Model Letter for Self-Certification that any individual can use.
