    Hi All,
    Does anyone have a form for an employer to document when former employees refuse the rehire offer? Also, how does an employer properly document when the employees don't return to work and will not sign and return any forms or letters?

    Send an email informing the employee to return to work.

    The letter referred to isn't the critical one. The offer letter is crucial and must contain specifics as to title, location, hours, rate of pay, starting date, etc. if it does not and just says return to work, UIAB will not deem it a bona fide offer.

    I would also send a letter to new York State Department of labor. Not only will this put them on notice, And provide a record for you, it may also stop unemployment benefits from going to this person.

    Spelling mist-aches due to dictation are unavoidable.
