Hi, Christine,
I've had similar situations with client delinquencies and oversights much like yours. In my experience, contacting the Attorney General's office and explaining the circumstances often resolves the issue. They may ask for the delinquent review but if you plead you case, they may wave the requirement, subject to future compliance.
Good luck.

Subject: NYS Charities Bureau Review
In 2020 we prepared a Not For Profits Financials in order to do their 990. Their revenue received in 2020 was under $250K so we advised them that they did not need an annual review for the Charities Bureau. When doing the closing entries we accrued donations deposited in 2021 that were meant for 2020. When we made this accrual it made their revenues $254K. We did not even realize that this meant they needed a review. I know - an oversight on our part. This week our client got notification from the Charities Bureau that a review was not included with the CHAR 500 and to submit the review within 20 days or risk penalties. Has anyone ever had this happen to them and is their a defense I can use to save my client penalties?
